Passport Services
The Office of Special Services is an authorized Passport Acceptance Agent. Appointments are required to obtain a passport. Please call (708) 403-6188.

  • Certified Birth Certificate or Naturalization Certificate
  • Photo Identification
  • Two Passport photos

Two separate checks or money orders are required!

Processing Time
Regular service is estimated at four to six weeks. Expedited service is estimated at two to three weeks and includes additional fees.

Additional Information
Children applying for a passport who are under 16 years of age must be accompanied by both parents.
