Special Olympic Medical Form
To participate in any Special Olympic program a valid medical form must be on file by the first meeting date of that program. Forms must be signed by a doctor and filed at the recreation department. Special Olympic Medical forms are valid for two years after date of doctor's signature. Medical forms must be valid from the first practice date through the state competition for any Special Olympic sport.
