The Short Form Inspection Request is used for the following permit types:

Above Ground Pools	Porch
Alarm	Reroof
Deck	Shed
Fence	Siding, Gutters, Fascia
Gazebo	Sky Lite
Hot Tub	Windows, Doors
Lawn Sprinkler	Other Minor Permits

The Long Form Inspection Request is used for the following permit types:

Basement	Commercial New & Remodel
Demolition	Construction & Sales Trailers
Garages	Fire Protection Inspections
In Ground Pools	Residential New & Remodel
	Special Events

The Electrical Service Inspection Request Form is used for all Electrical Service inspections. 

The Sewer, Water, Sump Inspection Request Form is used for all sewer, water, and sump inspections and also for sewer repair permits.

The Concrete, Deck/Post Hole Inspection Request Form is used for Concrete and Deck or Post Hole Inspections.  Concrete Inspections can also be requested using the Long Form Inspection Requests referenced above.
